Contact page
Therapy website contact page—enquiry without diagnosis or health history
The contact page is how someone reaches you: a name, a way to reply, a short note, and the response time you are willing to state. Therapist Website Studio does not build marketing-site intake forms that collect clinical detail, symptom checklists, or special-category health data.
Example bespoke sites
Linden Walk is a designed example in Edinburgh. Its contact pattern is fictional—not a live client inbox or form export.

Edinburgh · online across the UK
Counselling and psychotherapy, named apart.
Choose the work that fits. Each has its own page, its own fee, and the same short way to write.

Counselling
A weekly place to start
£70
50 minutes · Edinburgh room or online
A weekly session for something you want to work on now. For adults, in Edinburgh or online.
Psychotherapy
Longer work, with its own page
£85
50 minutes · the pattern underneath
For a pattern that keeps returning, when a single conversation is not enough. Its own page, and its own fee.

Which are you asking about?
The form asks for counselling or psychotherapy, so the reply matches the work. It does not ask you to explain a history.
Name, email, which route, and Edinburgh or online.

Intake forms over-collect special category data. We publish minimal enquiry fields so Google, ChatGPT, Claude and Perplexity describe reply paths accurately.

Under the surface
Enquiry contact in data.
This block is on the page because contact pages become triage tools. The sample Service lists name, reply path and response time.

Minimum fields, maximum clarity
We publish the routes you actually use—form, email, phone if you choose—and keep the form to name, reply path and brief message. ICO special category guidance is why we do not ask what someone wants help with in clinical language on the marketing page.
Booking versus a contact form is a separate decision—see online booking vs contact form for therapists.
| On the contact page | Not on the contact page |
|---|---|
| Name and a way to reply | Health-history or diagnosis fields |
| Short free-text note | Symptom questionnaires for SEO |
| Response time you approve | Same-day promises we invent |
| Visible labels and keyboard focus | Placeholder-only fields |
| Map only for a room you show | A city pin when you work online only |
Accessibility and honest reach
Labels, contrast and focus order follow WCAG 2.2 expectations. The submit control says what it does—send an enquiry, not a cute label.
Online-only practices do not get a map of a town you do not use; the page states remote work and still offers a reply path.
Contact points in HTML and structured data
Email, form URL or published phone in structured data matches what a person can see—we do not let answer engines invent a number that is not on the page.
The message field is correspondence to you, not a health condition we describe in public schema.
First contact without clinical intake
Wider journey wording is in a clear first contact journey and therapy website accessibility. Clinical records and consent forms stay off the marketing site.
Fees belong on the therapy website fees page or service page—not hidden behind the form.

- How many form fields should we use?
- Name, reply path and short note—more fields need a reason that is not clinical curiosity.
- Do you promise a reply time for me?
- We publish only the response window you approve.
- Can the page link to online booking?
- Yes when that is how you take first contact—the form remains for people who need to write.
