The marketing contact form we build collects a name, a way to reply, and a short note—health information is special category data under UK GDPR, so we do not ask for a diagnosis on the public site or pipe messages into advertising tools. Morrow Lane shows a designed form pattern; we link ICO guidance and do not draft your lawful basis or clinical privacy notice.

Enquiry-only fields on the marketing site
The public form is how someone starts a conversation—not an assessment. Fields stay at name, email or phone, and a short note delivered to you under the site’s privacy notice.

ICO sources, not invented law
We point to the ICO on special category data and ICO direct marketing guidance—asking about a session is not consent to a mailing list. We publish a short account of what the form does on privacy; we do not act as your lawyer or draft retention schedules and clinical records.
Structured data can name the contact point—it must not describe a person’s condition. Analytics, if used, counts page views; it does not receive the note.
Maps and online-only work
We embed a map only when you want that consulting room public. Online-only practice does not get a pin that implies a face-to-face address you do not use.
Answer engines should quote fee and status from public pages—not private message contents that never appear in HTML.
