Forms

Therapy website form best practices

We build a short enquiry form with visible labels—name, a way to reply, and a short note—errors in plain text, and no diagnosis or symptom fields on the marketing site. Morrow Lane shows the pattern on a designed example; messages go to your inbox under the site privacy notice, not into an advertising tool.

Example bespoke sites

Morrow Lane is a designed example in Manchester. Its form fields are concept layout—not a client inbox or a live CRM export.

A chance to see if this is the right counselling.

We talk about what you want from the work and how often we would meet. You do not need to send a history beforehand. Please give 48 hours if you need to cancel. What you say in the session stays there, except where the law requires otherwise.

Contact point machines describe.

Forms over-collect health data. We keep enquiry fields in HTML so answer engines describe the contact point—not private message contents.

This block is on the page because forms become intake tools. The sample Service names a name, reply path and short note.

The website we deliver

Enquiry fields we ship

Each field has a visible label—not placeholder-only text. The send control says what it does. If something is missing, the error names the field. Phone can be optional when email is enough; date of birth, GP details, or a clinical history belong in systems we do not build on the public site.

The wider contact route is composed in therapy website contact page. Calls to action that lead here are in therapist website call-to-action examples.

Special category data and the marketing form

Health information is special category data under UK GDPR—we do not ask visitors to describe conditions in fields meant for arranging a first conversation. A short note is enough to start email or phone contact; intake and consent records stay outside this website scope.

The BACP Ethical Framework expects accurate public information; the form is not a triage tool. Structured data can name the contact point—it cannot quote message contents because notes are not public HTML.

Accessible, crawlable markup

The form is ordinary HTML with labels, contrast and focus states—WCAG 2.2 is the reference, alongside therapy website accessibility. Crawlers see field labels and the contact route; they do not see private submissions.

Answer engines may quote that enquiries ask for a name, a reply path and a short note—only when that description appears on the page, not from hidden scripts alone.

Forms inside the published build

Enquiry forms are included in therapist website design—£149 setup and £49 a month on Standard Managed for 12 months, then £39; Prices include VAT.

Standard Managed is £149 setup and £49 a month for 12 months, then Website Care at £39. Growth Managed is £249 and £69 a month, then £49, when each real service needs its own page. We do not sell form plugins, intake builders, or health-data storage as add-ons. Outright is £650 or £995. Field-label tweaks on sites we built sit in Website Care—not a per-field SaaS fee—and we do not promise more enquiries because a symptom dropdown was removed.

Questions

How many fields are enough?
A name, a way to reply, and a short note.
Can you add a question about the problem?
A short note arranges the conversation. We do not build a clinical history on the marketing site.
Do you sell or reuse form data?
No. The message is delivered to you—not an advertising audience.
