Google image guidance and WCAG both want a text alternative that says what is shown—a portrait with your name, a consulting room you are willing to show, decorative lines left empty.
Alt is not a place to repeat “therapist near me”. If you have no photograph yet, the page can still launch—we do not fill the gap with a stranger from stock.

Portraits, rooms and maps
Portrait ethics and crop choices overlap therapist directory photo guide and photos and imagery—the site image is yours to approve, not copied from a listing.
